This section provides access to a range of important forms that parents and guardians are asked to complete.
Parents can download forms for current excursions and or permission slips for any events that are occurring at the college. These will change according to our calander.
Medical Forms
The following forms are required to be completed by parents of students with specific medial conditions including asthma and seizures.
Image Disclaimer Form
This form provides the opportunity to parents to provide permission for their child’s image to be published for College purposes.
ICT User Agreement
This form is required to be completed to enable students to use the computers and online learning opportunities at the school and sets out their responsibilities in using these tools.
Smart Rider Form
Students are entitled to apply for a SmartRider to receive the Student fare on public transport.
